Elective Surgeries Comprising 50% of Hospital Revenues Come to an Abrupt Halt. Laparoscopic Devices Sales Drops by -3.1%

The global market for Laparoscopic Devices is expected to slump by -3.1% in the year 2020 and thereafter recover to reach US$16.8 billion by the year 2027, trailing a post COVID-19 CAGR of 6.5% over the analysis period 2020 through 2027. Fears over the spread of COVID-19 infections & shortages of P.P.E are resulting in postponements and cancellations of surgeries, which make for a major part of hospital revenues. As infections continue to spiral, emergency reallocation of healthcare resources to fight the pandemic has sharply reduced hospital capacity available for non-COVID care. In other words, due to the effect of COVID-19 many non-cancer and non-urgent procedures have been put on hold so that the medical and paramedical staff could be reallocated to face the emergency situation. In addition, there have been many concerns that the minimally invasive surgeries could carry the virus risk in operating theatres during surgeries. Surgical smoke is a well-recognized hazard in the operating room. The environment created by pneumoperitoneum for laparoscopy creates a stagnant volume of heated gas in the abdominal cavity which may subsequently allow for a concentrated aerosolization of the virus. In the year 1996, the presence of cell-sized fragments and breathable aerosols was identified in the cautery smoke that was produced during the laparoscopic procedures. The size of these particles ranged between 0.1 and 25 ?m. However, this size was dependent on the device used. An aerosol includes the suspending gas along with the liquid and solid particles of the gas, which is CO2 in case of laparoscopy.

Although no contamination of surgical team has been recorded, it has been identified that papillomavirus DNA and hepatitis B virus are present in surgical smoke. The particle concentration in the smoke produced is higher in case of laparoscopy compared to open surgery. It is still unknown if SARS-CoV-2, similar to other viruses, may be found in the surgical smoke during laparoscopic procedures. As the knowledge regarding the coronavirus is very limited, the possible risks that arise due to operating an asymptomatic patient for medical professionals are not clear. Nevertheless, as a safety precaution elective surgeries have been postponed. General surgeries comprising of mostly minimally invasive procedures including laparoscopy have recorded sharp declines. Gynecological laparoscopic surgeries have taken a massive blow given the high risk of COVID-19 transmission via contaminated aerosols from CO2 leakage and smoke created by surgical energy devices. The decline in laparoscopic procedures is expected to result in over US$349.8 million in revenue losses for laparoscopic device manufacturers.

In the post COVID-19 period as surgeries begin to resume, demand will grow for smoke evacuation devices with high efficiency filters that minimize exposure of operative staff to aerosolized particles. The fear that surgical smoke may contain viable SARS-CoV-2 sheds focus on commercially available smoke evacuation system. The MEGADYNE™ Smoke Evacuator is designed to remove plume and potentially hazardous materials for a clear field of vision and aid in pneumoperitoneum release. The system can also evacuate CO2 gas towards the end of the procedure by using a filtering system. Suction devices such as the MEGADYNE™ ULTRA VAC™ Smoke Evacuation Pencil aids in effective elimination of aerosolized particles. Long-term outlook for the market remains promising with continuous technology development helping meet the changing requirements in the OR and offering surgeons the ability to achieve exceptional patient outcomes at reduced costs. Over the years, significant developments have been made in the instrumentation space that has brought laparoscopic surgery to a whole new level. The recent years have also witnessed a significant improvement in the techniques focused on minimizing the number of ports to achieve better cosmetic results. Among the new techniques, single incision laparoscopic surgery (SILS) and Natural Orifice Tran luminal Surgery (NOTES) are gaining ground due to improved outcomes of minimal access surgery. This is paving way for the development of solutions aimed at enhanced ergonomics, excellent visualization, reduced patient scarring and eliminating the need for port suture closures. The market will benefit under the influence of accessibility to HD (high-definition) systems and utilization of latest technology such as co focal micro endoscopy, narrow band imaging and advancements in the areas of video scopes and fiber optics. Higher requirement for functional efficiency and overall endoscopic equipment and accessory tools, specifically flexible endoscopes is also likely to push the market. The introduction of various tools, such as improved imaging systems, 3D laparoscopy systems, multiple robotic devices, temperature-controlled devices, and surgical instruments that offer enhanced articulation and rotation are expected to expand the use of laparoscopic techniques in future.
