Traveler's Diarrhea (TD) - Disease Understanding

Traveler's diarrhea (TD) is defined as ≥3 unformed stools in 24 hours accompanied by at least 1 of the following: fever, nausea, vomiting, cramps, tenesmus, or bloody stools (dysentery), in a traveler from a developed country visiting a less developed country. It is usually a benign self-limited illness lasting 3 to 5 days (mean 3.6 days).

Classic travelers' diarrhea is defined as at least three loose to watery stools in 24 hours with or without one or more symptoms of abdominal cramps, fever, nausea, vomiting, or blood in the stool. Mild to moderate diarrhea is one or two loose stools in 24 hours with or without another enteric symptom. The median time to onset is six or seven days after arrival. Although the diarrhea often resolves spontaneously over three or four days, up to a quarter of affected travelers need to alter their plans, interrupting their holiday or business activities.

TD is a clinical syndrome that can result from a variety of intestinal pathogens. Bacterial pathogens are the predominant risk, thought to account for up to 80%-90% of TD. Intestinal viruses usually account for at least 5%-8% of illnesses, although improved diagnostics may increase recognition of norovirus infections in the future. Infections with protozoal pathogens are slower to manifest symptoms and collectively account for approximately 10% of diagnoses in longer-term travelers.
